Citomegalovírus congênito: avanços no rastreamento pré-natal, diagnóstico fetal e perspectivas terapêuticas

Introdução: A infecção congênita pelo citomegalovírus (CMV) é a infecção congênita mais prevalente no mundo e representa importante causa de morbidade neonatal, sendo a principal etiologia não genética de perda auditiva neurossensorial e uma das principais causas de comprometimento do neurodesenvolvimento infantil. Apesar dos avanços diagnósticos e terapêuticos, persistem desafios relacionados ao rastreamento pré-natal, ao diagnóstico fetal precoce e à padronização das estratégias de manejo. Objetivo: Analisar as evidências científicas sobre os avanços no rastreamento pré-natal, diagnóstico fetal e perspectivas terapêuticas da infecção congênita pelo citomegalovírus. Métodos: Trata-se de uma revisão integrativa da literatura conduzida conforme as recomendações do PRISMA. A busca foi realizada nas bases de dados PubMed/MEDLINE, SciELO e Biblioteca Virtual em Saúde (BVS), utilizando descritores relacionados ao citomegalovírus congênito. Foram identificados 48 estudos, dos quais 12 atenderam aos critérios de elegibilidade e compuseram a amostra final. Resultados: Os estudos demonstraram avanços significativos na utilização da reação em cadeia da polimerase (PCR) para o diagnóstico materno, fetal e neonatal, além da ampliação das evidências sobre o uso do valaciclovir durante a gestação e do valganciclovir em recém-nascidos sintomáticos. Entretanto, permanecem desafios relacionados à ausência de programas universais de rastreamento, às limitações da sorologia materna, à heterogeneidade das diretrizes clínicas e à necessidade de identificação precoce dos recém-nascidos em risco para sequelas auditivas e neurológicas. Conclusão: O diagnóstico precoce, aliado ao acompanhamento multidisciplinar e ao uso racional das terapias antivirais, representa a principal estratégia para redução das complicações da infecção congênita pelo CMV. O fortalecimento das políticas de prevenção, da educação em saúde e das pesquisas voltadas ao desenvolvimento de vacinas e novos biomarcadores poderá contribuir para minimizar o impacto da doença na saúde materno-infantil.
