Citomegalovirus congénito: avances en el cribado prenatal, el diagnóstico fetal y las perspectivas terapêuticas

Introducción: La infección congénita por citomegalovirus (CMV) es la infección congénita más frecuente en el mundo y constituye una importante causa de morbimortalidad neonatal, siendo la principal causa no genética de pérdida auditiva neurosensorial y una de las principales causas de alteraciones del neurodesarrollo infantil. A pesar de los avances diagnósticos y terapéuticos, persisten desafíos relacionados con el cribado prenatal, el diagnóstico fetal precoz y la estandarización de las estrategias de manejo. Objetivo: Analizar la evidencia científica sobre los avances en el cribado prenatal, el diagnóstico fetal y las perspectivas terapéuticas de la infección congénita por citomegalovirus. Métodos: Se realizó una revisión integradora de la literatura siguiendo las recomendaciones PRISMA. La búsqueda se efectuó en las bases de datos PubMed/MEDLINE, SciELO y Biblioteca Virtual en Salud (BVS), utilizando descriptores relacionados con el citomegalovirus congénito. Se identificaron 48 estudios, de los cuales 12 cumplieron los criterios de elegibilidad y fueron incluidos en la revisión. Resultados: Los estudios demostraron importantes avances en la utilización de la reacción en cadena de la polimerasa (PCR) para el diagnóstico materno, fetal y neonatal, así como un aumento de la evidencia sobre el uso de valaciclovir durante el embarazo y valganciclovir en recién nacidos sintomáticos. Sin embargo, persisten desafíos relacionados con la ausencia de programas universales de cribado, las limitaciones de la serología materna, la heterogeneidad de las guías clínicas y la necesidad de identificar precozmente a los recién nacidos con riesgo de secuelas auditivas y neurológicas. Conclusión: El diagnóstico precoz, asociado al seguimiento multidisciplinario y al uso adecuado de terapias antivirales, constituye la principal estrategia para reducir las complicaciones de la infección congénita por CMV. El fortalecimiento de las políticas de prevención, la educación sanitaria y las investigaciones orientadas al desarrollo de vacunas y nuevos biomarcadores contribuirán a disminuir el impacto de esta enfermedad en la salud maternoinfantil.
