Comparison of Clinical Outcomes Between Sleeve Gastrectomy and Gastric Bypass

Obesity is a multifactorial chronic disease associated with important metabolic and cardiovascular complications, as well as reduced quality of life. Among the available therapeutic strategies, bariatric surgery stands out as the most effective treatment for sustained weight loss and control of obesity-related comorbidities. This study aimed to compare the clinical, metabolic, and postoperative outcomes of sleeve gastrectomy and Roux-en-Y gastric bypass through a systematic literature review. The bibliographic search was conducted in PubMed, Scopus, Embase, and Web of Science databases, initially identifying 247 relevant studies, of which 16 were included in the qualitative analysis after eligibility criteria were applied. The analyzed studies demonstrated that both techniques promote significant weight loss and improvement of obesity-related comorbidities. However, gastric bypass showed superiority regarding sustained weight loss, remission of type 2 diabetes mellitus, dyslipidemia control, and improvement of gastroesophageal reflux disease. On the other hand, sleeve gastrectomy demonstrated lower technical complexity and lower incidence of some late complications. It is concluded that both techniques are effective and safe, although they present distinct characteristics that should be individualized according to each patient’s clinical profile.
