Abstract

Peritoneal carcinomatosis of appendiceal and colorectal origin represents a therapeutic challenge due to the high frequency of recurrence and the limitations of systemic treatment alone. This review aimed to analyze the role of cytoreductive surgery (CRS) combined with hyperthermic intraperitoneal chemotherapy (HIPEC) in the treatment of these malignancies, considering clinical outcomes, survival, and prognostic factors. Fifty-seven studies were identified, of which nine were selected for analysis. The findings indicated that CRS combined with HIPEC may provide disease control and survival benefits in carefully selected patients. Complete cytoreduction emerged as one of the main factors associated with better outcomes, while the extent of peritoneal disease, primary tumor origin, lymph node involvement, and postoperative complications also influenced prognosis. Evidence regarding additional systemic chemotherapy was heterogeneous, particularly in appendiceal tumors, highlighting the need for individualized treatment strategies. In conclusion, CRS and HIPEC represent a relevant therapeutic approach for selected patients, provided that treatment is preceded by multidisciplinary assessment and performed at experienced centers.
