Abstract

Introduction: Advanced Parkinson’s disease is a progressive neurodegenerative disorder frequently associated with disabling motor complications, including tremor, rigidity, bradykinesia, dyskinesias, and motor fluctuations that often become refractory to conventional pharmacological therapy. In this context, Deep Brain Stimulation (DBS) has emerged as an effective neuromodulation strategy for symptom control and functional improvement. Objective: To analyze the scientific evidence regarding the efficacy of deep neuromodulation in patients with advanced Parkinson’s disease, emphasizing its effects on motor symptoms, quality of life, and management of treatment-related complications. Methods: An integrative literature review was conducted using studies indexed in PubMed, including clinical trials, systematic reviews, and meta-analyses evaluating DBS targeting the subthalamic nucleus and globus pallidus internus in patients with advanced Parkinson’s disease. Results: The analyzed studies demonstrated significant improvements in motor function, reductions in OFF periods, decreased levodopa-induced dyskinesias, and enhanced quality of life. Additionally, DBS was associated with a reduction in dopaminergic medication requirements, particularly among patients undergoing subthalamic nucleus stimulation. Long-term follow-up studies indicated sustained clinical benefits with an acceptable safety profile when performed in specialized centers. Conclusion: Deep neuromodulation through DBS represents an effective and safe therapeutic option for patients with advanced Parkinson’s disease. Current evidence supports its role as an important adjunct to conventional medical therapy, contributing to improved motor control, functional capacity, and overall quality of life.
